Remove both sides and compare bearing numbers, order new incase offending one is incorrect?Bearings do not come loose by themselves. I suspect it is the wrong bearing.
It looks like the washer is there. Livs313 confirmed it.McAlpine identified the problem. Has the OP implemented it yet?
Just for clarification, are you saying the whole hub needs to be replaced? Is there any reason why new bearings and races can't be installed in the old hub?The answer is in post #12
