The engine starts with a sharp jolt and vibrates until it worms up. What could be the problem? I live in Las Vegas Nevada and the temperature outside is now in the 70s but when it gets colder the engine vibrates even worse.

The car has 120,000 miles and the front engine mounts look just fine, no rocking of the engine when going from forward to reverse or pushing the throttle down slightly when the brakes are activated. I am unable to easily see the transmission mount because of the bottom engine splash cover. That's what I thought it was. Thanks for your reply.

If mounts are good, check injector correction values and do leak back test. Very often shaky idle is caused by worn injector, they are temperature sensitive too.

check your swirl flap valve linkage.

Unfortunately, I was rear ended and the CDI is in the bodyshop for now (if not totaled). Regardless I plan on keeping the vehicle, so I will reply to you as soon as I get it back and check it, which may take up to 2 weeks.

I had the same problem happen a couple years ago, rear ended by a Semi on the highway. $8500 of damage, and the insurance company didn't total it! :D

It wasnt toataled, only 1.15K in damage, my car should be fixed by monday, so I will report back by tuesday night about the swirl flaps.

The whole situation just gave me a headache though.
1. Dealing with uninsured motorist
2. Uninsured party falsifying report
3. Uninsured party calling my insurnce for their medical bills on a 10mph accident.
4. Police got the whole story wrong (the scenario they described is completely impossible with the damage on the vehicles)

I will be putting my dashcam on now, and the insurance still found other party at fault and denied their claims..
Still a headache though, people should have insurance here in Illinois.

Is it not illegal to drive without insurance in Illinois?

One thing that could be done too, is to call your insurance company and give an on-record crash report as it happened to you. It's legally permissable in court and gives you the upper hand should you ever need to threaten legal action.

Swirl Flap Linkage is attached, no problem there.

Here are my symptoms:
-Vibrations up until the car warms up to 85 degrees in D,R (can't really feel when driving, only when fully stopped on the brakes)
-A white puff of smoke when cold starting (dissipates quickly, smells of diesel I believe)
-Excessive sut when flooring it.
-Can feel the engine vibrating when hand is put on intake manifold/engine

P.O mentioned that mounts have been replaced within the last 2K miles (Mechanic confirmed they looked new and in good shape)
I believe the vibrations did get worse as they weren't as prominent when I purchased the car about 8K miles ago.

Judging by the symptoms, i think it might be injectors. whether its the injector body itself or the nozzles, that's for a professional to discern.

Justin, have you had this car the whole time or is this a recent purchase? The mileage is about right time for mounts and these are fluid-filled mounts. There would be residue of this brown silicone staining the subframe. Your symptoms are very common on 100-120,000 mile cars. If you don't have a way to read codes perhaps seeking out a specialist familar with CDI mercedes would be a good move.

I have had the vehicle for the past 1/2 year.
PO said Mounts Are New, they also do look new.

NEW UPDATE ( WITH FINDINGS!)
I did a leak-back test, and all injectors return the same amount of fuel. HOWEVER, The diesel is Green on cylinder #6, and it gets more brown on each cylinder till it is fully brown on cylinder #1 (closest to the radiator)!

The return lines spit out green

What even is this? What could be the issue??

FOUND THE SOLUTION!

I had similar symptoms as OP. When Chicago got colder, my car started to vibrate a lot more, which led me to suspect the engine mounts nearly 100%.

In Short: The previous owner installed gas engine mounts in the CDI.

The Previous Owner replaced the mounts with gas engine or 07+ BlueTech engine mounts (didn't care enough to look into it that far). Although they looked brand new and in good shape (They were literally just a couple months old), they caused the vibrations.
I replaced with brand new Lemforder 06 E320 CDI Engine +Trans mount kit. After that, not a single vibration!
