Using either a plastic wedge or a covered flat screwdriver, you can pop off the triangular shaped black plastic cover on the inside of the door. Remove the cover and you'll see the screws that hold the mirror assembly onto the door.

My pivot arm broke too.

I got one off a wreck. As he said pop off the plastic
triangle inside the car. There are three screws. Undo these. The handle on the pivot arm has a little mechanism that hold it on the metal. If you were in the car sitting, you would push towards the mirror, since yours is broken off like mine, you can see this much easier. The hard part will be popping off the plastic housing to replace the innards. I'm affraid to mess up my mirror, so I'll practice on my salvage part.
