You've checked the codes on the OBDII port and pins 7 and 8 on the 38 pin? Could be the harness. What exactly is it doing?

My S420 had a rough idle that lasted almost to the point where I was about to give up and just live with it. Replaced everything from CAT, spark plugs, wires, fuel injector, to checking into throttle control. Ended up found out that the timing chaing needed to be replaced. Engine was running out of time. Not sure on the 6cyl engine whether it would be the same case. Good luck.

A slipped chain is much more likely on a 119 motor than a 104 motor. Timing chains on a 104 motor seem to be very reliable.
