My 99 E300 (202,000 mls) will run rough with more smoke than normal when I first start the car after sitting overnight. I have no CEL. After about 30 seconds, it will run perfectly as it will all day. Could it be getting air in the system or perhaps an injector is leaking down?

Need advice from the W210 gurus!

Injector leaking down is more plausible. Might be worth your time to rip out the injectors for rebuild using monark nozzles.

Yeah, smoke at startup usually means excess fuel in the cylinder and the smoke is it burning off. You see this when you have glow plug issues, but that will trip the CEL. That is kinda odd though. I have 2 99 e300's , both over 250K and I have never had an injector problem. I have replaced the injection pumps in both , because I ran WVO in them for a time. I don't recommend that, by the way. It works great in the old 1980's 5 cylinder diesels, but not newer 6 cylinder. I have pulled the injectors out of one of mine before, it is not difficult. Rebuilding them maybe worth a shot. I would buy a new section of injector fuel, the line that jumps from injector to injector, then back into the fuel system. That line gets brittle and leaks.

Thanks, orb353. Do you know of any guides to removing the injectors? I know how on the W126 & W123 models, but I haven't had to work on my 99.
