When it is cold out, my car starts very hard, sometimes taking several tries. When it does start, it runs very rough for a few seconds and emits acrid smoke. The colder it gets, the worse the cold start problem. No codes, though. My mechanic (who replaced all six glows two years ago) finds no codes, and the temperature sensor reads fine. I'm wondering if the problem is the glow plug controller, but wonder if something like that could be faulty and NOT throw a code?

My bet is your mechanic put in 12V plugs instead of the 5V Beru plugs. Have him pull one plug to see what voltage is marked on it. FYI, with the 12V glow plugs, the cdi will not throw any codes.

Sounds like that is the problem. Thanks for letting me know that. I found the thread on this issue.
