Anybody have the accessory rear entertainment system (single screen)? Do you lose the cupholders? Any negatives?

Yes, you'll loose the cup holders behind the front center console (on the bottom) and the sound can be heard through a pair of wireless headphones ( a plus), and be heard through car speakers via FM modulator (a minus, with static and loose of clarity). Dealer installed costs around $2,000 ($1,000 parts, $400 labor).
For same price you may be able to get aftermarket dual dvd monitors behind headrests. Check out Visualogic or Rosen systems. If you have '07 model, the front headrests are active and self-restraint, both Visualogic and Rosen make special units (mounted monitors behind headrests) for these.
Just had my dealer install the singel screen rear entertainment system. It works very nicely, but I am not sure the dealer has installed it correctly. When the screen is folded down it covers the vents partially and there is a gap between the DVD player and the plastic casing. I accidently dropped the remote control in that slot and it fell into a deep hole .... took some wire to 'fish' out the remote.
Does anyone have the installation manual and operating manual? My dealer can't seem to find it.
There should not be an empty slot like that. There is a plastic part that covers it. Perhaps installer forgot/ignored that. So yes, go back and have it fixed.
