how to test/adjust vacuum modulator

After changing the vacuum levers (a la Diesel Giant) I have now come to the conclusion that my hard shifting is due to the vaccum module. I bought a new one & will be changing it this weekend weather permitting.
My problem is the one that is there now was put in by a mechanic that I don't have a lot of confidence in so I can't go with the old "this is the way the old part went in so I'll do the same for the new one" approach.
I searched this forum & can find anything on point.
does anyone know how this gizmo is supposed to go in?
also the part comes with a little metal tool that seems to be desined to adjust it; how's that go?

Did you test the vacuum system yet? You need a mity-vac to do this. A leaking vaccum control valve can cause probelms, as can cracked rubber connectors.

Test the system and plug leaks before messing around with the tranny.

I meant to title my enquiry "how to replace/adjust vaccum module" my bad!
I did the vaccum testing & it all checks out fine.

So how does one change the thing? & what is that little tool for ?

also,
To better describe my problem, I noticed today that she shifts fine into 2nd & 3rd pretty well, i.e. at a proper time & fairly smoothly, in other words acceptable.
it's getting into 4th that she clunks harshly.
For example:
When i'm in commuter traffic cruising at 40MPH if I kick it a little, she'll down shit like normal, but when she shifts back up to 4th it's clunk city. I know one way to avoid the annoyance is to drive in 3rd but that's not practical for commute since I typically drive 25 miles between 35 and 60MPH
