Its easy to put the blame on the computer or software so the school took the easy way out and said," The school has apologized for the mistake, which they said was down to an error on the computer database."

Error in the database is caused (99% of the time) by human/operator errors. The school failed to provide appropriate control over the process starting with, (1) data input control (2) data processing control and (3) data output monitoring control. Absence of basic controls on mission critical application could have disastrous result.

Luckily, in this situation, it's only a notification letter. I know of a case wherein an inmate was set free due to data input error.

So the parents should have exhumed the body and delivered the casket? Talk about digging up an appropriate response.

While I like cmitch's suggestion I think a small box of ashes, delivered the next morning for first bell with a note of apology from the mum would have been nearly as good.
