I agree with you 100% on these things as being the simply monstrous and unforgivable failures. Another thing that should be on your list was security to close the borders with Iran and Syria to prevent an influx of foreign fighters. They're all part of what is called "phase 5", which deals with security and reconstruction after the government is toppled. There simply was no phase 5. I'll echo FTL's suggestion to read Fiasco by Thomas Ricks. It was a great book, largely because it didn't address the reasons behind the invasion - it only focused on what happened once the decision to invade was made.

The invasion - phase 4 - was successful because the objective was met. The objective was removing Saddam Hussein and his government. Everything after that point was a complete and utter clusterfuck. Rumsfeld chose to ignore the "Powell Doctrine" of using overwhelming force to crush your opponent once the decision to go to war has been embraced. If Powell was in charge there would have been 300,000 pairs of boots on the ground, and there would have been a plan for where things go once the US effectively owned Iraq.

Here is a fact: Many Arabs strongly believe that some elements in Israel aim to reclaim it's biblical territory which expand well beyond present Israel. So, when they see us doing Israel's bidding what do you think they are thinking? We're implementing democracy in the ME? that's all, nothing to watch, move along and things will get better

Surely you can see how an effective temporary occupation that supports internal stability in Iraq is counterproductive toward a goal of permanent occupation. I believe it's a mistake to assume that the architects of this fiasco are/were blatantly stupid. All is going according to plan. The only failure by their measure would be our extraction from Iraq -- for any reason.
