2000 E320 Wagon 4Matic - Engine Rattle; Top of Motor?

Hello all, I have been a lurker in these forums for a year. Reading and rarely posting. I sold my 96 e320 a while back and finally traded from my $1500 Honda Civic to my much sought after e320 4matic wagon.

Anyway - My car has a loved 166,000 miles on it and is my daily driver. When I bought it, it needed front shocks (not fun, but luckily i got the compressor needed for free!), drivers window regulator changed and rear window regulator changed. All of which I have done with the help from the forum.

So here's the recent issue that is driving me nuts -

There is this "rattle" that is metallic in sound and somewhat consistent. It makes the noise only at idle from what I can tell... So I thought, Water Pump, Tensioner, Idler Pulley? Alt? Crank Pulley? So I thought "Screw it" and pulled the belt off all together, started it and viola the noise is still there! So I read some more posts, and bought a Stethoscope and listened to everything I could touch it too... I cannot track it. It seems like its at the top of the motor, dead center maybe? Sounds like its coming from the Secondary Injection Air pump maybe? But I do not think that is it either because I couldn't isolate it with the stethoscope either. I even unplugged it to see if it would stop but no luck. Yes I know it only works on cold startup.

I don't think its the cat's
I know it's not pulley's, pump's or?
I scoped the valve covers and I don't hear the noise on either side.

Can anyone give me an idea? I even put the scope on the Exhaust Manifold and the passenger cat. nada.

It's driving me crazy. I love this car, and I know my car is making a noise that I do not hear from other cars of the same make, model and vintage.

I don't have a clue what might be causing this one.
